Age The age of the patient
Sex Male or female. Enter as appropriate.

Low Alcohol Use Between 1 and 14 units per week.
Moderate Alcohol Use Between 15 and 21 units per week.
Hazardous Alcohol Use More than 21 units per week.
Rheumatoid arthritis Enter yes where the patient has a confirmed diagnosis of rheumatoid arthritis. Otherwise enter no.
Systemic Lupus Erythematosus Enter yes where the patient has a confirmed diagnosis of systemic lupus erythematosus. Otherwise enter no.
Inflammatory Bowel Disease Enter yes where the patient has a confirmed diagnosis of Inflammatory Bowel Disease. Otherwise enter no.
Chronic kidney disease stage 3 Patients with chronic kidney disease stage 4 or 5 were excluded and the model should not be applied to them
Drugs If currently prescribed.
Current smoking Enter yes or no depending on whether the patient currently smokes tobacco.
Current thiopurine dose For patients prescribed azathioprine, please enter the daily aza dose and the daily mercaptopurine equivalent dose will be automatically calculated For patients prescribed mercaptopurine, please enter the daily mercaptopurine dose. Please do not enter both daily azathioprine and mercaptopurine doses at the same time.
Blood test abnormality in 6 months of primary care prescription Defined as neutrophil count <2x109/l or total leucocyte count <4x109/l, or platelet count <150x109/l or raised transaminase (ALT and/or AST >35 IU/l)
