Age The age of the patient
Sex Male or female. Enter as appropriate.
Weight This should be entered in kg.
Height This should be entered in cm.
Low Alcohol Use Between 1 and 14 units per week.
Moderate Alcohol Use Between 15 and 21 units per week.
Hazardous Alcohol Use More than 21 units per week.
Current smoking Enter yes or no depending on whether the patient currently smokes tobacco.
Rheumatoid arthritis Enter yes where the patient has a confirmed diagnosis of rheumatoid arthritis. Otherwise enter no.
Connective Tissue Disease Include lupus, systemic sclerosis, myositis, small vessel vasculitis.
Chronic kidney disease stage 3 Patients with chronic kidney disease stage 4 or 5 were excluded and the model should not be applied to them
Drugs If currently prescribed.
Anti-epileptic drugs Include carbamazepine, levetiracetam, and sodium valproate.
Blood test abnormality in 6 months of primary care prescription Was defined as either cytopenia (neutrophil count <2x109/l, total leucocyte count <4x109/l, or platelet count <150x109/l) or elevated transaminase (ALT and/or AST >35 IU/l) during the first 6 months of primary-care methotrexate prescription. If methotrexate is always prescribed from a hospital or specialist clinic, this parameter should be evaluated during first six month of methotrexate prescription at a stable dose.
